Youth Photography Consent Form

Please sign, and tick the boxes below to confirm that you give permission for photographs and video recordings to be taken of your children. *

I give permission for my child’s photograph to be used in Jubilee Church London’s promotional materials, including printed and electronic publications.
I give permission for my child’s image to be video recorded for use in Jubilee Church London’s promotional videos, social media promotion and websites.
I give permission for images of my child to be used by the news media in printed and/or electronic form and stored in their archives.
I do not want my child’s image used in any publicity.

Occasionally, we may take photographs of children who attend the Youth Service held at Jubilee Church London. We may use these images on promotional materials, on websites, for promotion on social media, and on displays (i.e. exhibition stands). We may also make video recordings to be used for promotional videos, social media and for use on our website. We may, on very rare occassions also send images to the news media, if requested, and childen may appear on these images. When we submit photographs to the media we have no control on when, where, if, or how they will be used. To comply with the Data Protection Act 1988 we need your permission before we can take photographs or make any recordings of your child.
